Free Spay/Neuter Assistance Application The Free Spay/Neuter Fund was established to provide financial assistance for the purpose of spaying/neutering to individuals who are feeding and providing shelter for 6 or more outside cats. To be considered for financial assistance, individuals must meet and agree to ALL of the following conditions: 1.      A resident of Washington or Greene County in the state of Pennsylvania. 2.      Currently feeding and providing shelter for 6 or more outside cats. 3.      Make a commitment to continue to provide food and shelter. 4.      Consent to an on-site visit by a representative of Catnip Acres. 5.      Complete and sign the following application.

	By agreeing to our terms and meeting our requirements check the YES reply. You will be required to sign a HARD copy of this contract when you arrive at Catnip Acres.

	Please note that assistance is based on funds availability. Further, assistance is limited to covering the cost of spay/neuter, rabies vaccine, FVRCP vaccine, flea treatment, and earmite treatment. All services must be performed at Catnip Acres. An appointment is necessary and one will be scheduled for you.
